Job summary

Adara Home Health, Inc. is seeking an experienced RN Case Manager to provide direct patient care and oversee home health services in Minnesota. You will collaborate with physicians, families, and the health care team to develop goal-centered care plans and ensure quality and compliant care.

This full-time role requires travel to client homes (1–3 hours per visit) and on-call rotation. Competitive base salary of $70,000–$95,000 per year plus productivity bonuses, benefits, mileage reimbursement,

Qualifications

• RN license in Minnesota (without revocation/suspension)
• TB test or chest X-ray proof of negative testing
• Access to dependable automobile for multiple daily stops
• U.S. citizenship or valid alien work permit
• Background checks and screenings per state/federal requirements
• Two years’ healthcare facility or home care experience preferred

Responsibilities

• Reflect company mission and values while building professional relationships
• Assess clients, plan interventions, and evaluate care outcomes
• Collaborate with physicians and home health team to update plans of care every 60 days
• Provide client advocacy and safety plans, including abuse prevention
• Ensure documentation accuracy and payer/billing compliance
• Educate clients, families, caregivers, and licensed/unlicensed staff
• Provide direct care to clients with varying ages and needs
• Coordinate care with family, resources, and health team
• Provide clinical oversight and leadership to staff
• Manage financial aspects of home health services
• Maintain flexible schedule and participate in on-call process
