Job summary

Adara Home Healthcare in Denver area is seeking a compassionate Registered Nurse (RN) to provide skilled nursing care in patients’ homes under physician orders and care plans. You will collaborate with a dedicated clinical team to deliver high-quality, patient-centered care.

Responsibilities include assessments, care coordination, patient education, and thorough documentation. The role offers flexible scheduling and a supportive culture to promote work-life balance.

Qualifications

• Active RN license in good standing.
• Graduate of an accredited nursing program.
• At least 1 year of nursing experience (preferred).
• Strong clinical judgment, communication, and organizational skills.
• Current CPR certification.
• Ability to work independently and manage time effectively.

Responsibilities

• Deliver skilled nursing care to patients in their homes based on care plans.
• Conduct initial patient assessments and ongoing evaluations.
• Coordinate care and communicate changes with physicians and team members.
• Educate patients and families on care and health management.
• Maintain accurate, timely clinical documentation (including OASIS).
• Participate in care planning, case conferences, and quality improvement.
• Supervise LPNs, CNAs, and HHAs per regulatory guidelines.
• Support patients’ rehabilitative and preventive care needs.
