Job summary

Adara Home Health is seeking an RN Case Manager to provide direct client care and supervise home health services in the community. You will collaborate with clients, families, and the health care team to develop goal-centered plans, ensuring quality and financial considerations are addressed.

This full-time role requires travel to client homes, with a 1:1 care pace and coordination with a Clinical Supervisor. Minnesota RN license and reliable transportation are essential.

Qualifications

• Licensed RN in Minnesota; license not revoked or restricted.
• Proof of negative TB testing ( Mantoux, TB blood test, or chest x-ray).
• Access to dependable automobile for travel to client homes.
• Valid driver's license and auto insurance (personal vehicle use).
• U.S. citizenship or valid Alien Work Permit.

Responsibilities

• Oversee and direct home health services for clients.
• Develop and update care plans; review every 60 days or as changes occur.
• Provide skilled nursing care and clinical oversight to staff.
• Coordinate client care with family, caregivers, and health team.
• Ensure documentation is accurate, timely, and compliant with payer requirements.
• Educate clients, families, and unlicensed personnel.
• Manage scheduling and participate in on-call rotations.
