Adara Home Health, Inc. is seeking an RN Case Manager to deliver direct nursing care and oversee home health services in the community.

You will collaborate with clients, families, and the health care team to implement goal-centered plans, ensuring quality, regulatory compliance, and financial considerations are addressed. The role requires travel to client homes (1–3 hours per client) and entails shifts Monday–Friday with on-call rotation.

Qualifications

• Current RN license in Minnesota; license must be in good standing.
• Proof of negative TB testing (Mantoux, TB blood test or chest X-ray).
• Access to a dependable automobile for travel to client homes.
• Valid driver’s license and current car insurance for company travel.
• U.S. Citizenship or valid Alien Work Permit.
• Background checks including MDH and OIG List of Excluded Individuals and Entities.

Responsabilités

• Reflect company mission and values in professional relationships.
• Assess clients to determine care needs; implement interventions and evaluate outcomes.
• Collaborate with physicians and home health team to update care plans every 60 days or as needed.
