The role requires MN RN licensure, reliable transportation, and the ability to travel to client homes. Office travel and on-call rotations are part of the schedule, with a competitive base salary and benefits.

Qualifications

• Licensed RN in Minnesota without restrictions or revocation.
• TB testing/health clearance as required.
• Reliable automobile with valid license and insurance.
• Citizenship or work permit compliance.

Responsibilities

• Assess clients to determine care needs and implement plan of care.
• Coordinate with health care team and family for goals.
• Provide direct nursing care and clinical oversight to staff.
• Maintain documentation with payer/billing requirements.
