Job summary

Care Lync is seeking a Supports Coordinator for Early Intervention Services to help families access services, understand their rights, and receive coordinated supports for children from birth to age 6.

You will coordinate evaluations, IFSP development, and multidisciplinary planning, while promoting family empowerment and ensuring compliance and effective service delivery across providers.

Qualifications

• Bachelor's degree in one of the listed fields is required.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

• Provide ongoing service coordination for children birth to age 6 and their families.
• Serve as primary point of contact for families and coordinate service delivery.
• Coordinate evaluations and IFSP/FSP development and reviews.
• Educate families on rights and safeguards and promote empowerment.
• Monitor services to ensure alignment with IFSP/FSP and Medicaid/IDEA Part C.
• Maintain accurate records and complete documentation; collaborate with teams.
