Job summary

Care Lync seeks a dedicated Supports Coordinator (Early Intervention Services) to provide ongoing service coordination for children birth to age 6 and their families. You will help families access services, understand rights, and receive high-quality early intervention supports.

The role covers coordination of evaluations, IFSP/FSP development, and collaboration with teams to promote child development and family capacity in community settings.

Qualifications

• 1 year of experience working with infants, toddlers, or children with disabilities preferred.
• Knowledge of IFSP/FSP development and child milestones helpful.
• Experience coordinating services and advocating for families.

Responsibilities

• Serve as primary point of contact for families and coordinate and monitor service delivery.
• Coordinate evaluations and IFSP/FSP development and reviews.
• Educate families on rights and safeguards, promoting caregiver empowerment.
• Monitor services for alignment with IFSP/FSP and coordinate across systems.
