Job summary

Care Lync seeks a Supports Coordinator for Early Intervention Services to coordinate services for children from birth to age 6 and their families. You will facilitate IFSP development, monitor delivery, and educate families on their rights in the context of community-based supports.

Under supervision, you’ll collaborate with providers and families to ensure access to services, maintain documentation, and support transitions while complying with Medicaid and IDEA Part C guidelines.

Qualifications

• Bachelor's degree in one of the listed fields or related area.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

• Serve as primary point of contact for families and coordinate referrals.
• Coordinate evaluations, IFSP/FSP development, and reviews.
• Educate families on rights and safeguards; promote empowerment.
• Monitor service delivery and ensure alignment with IFSP/FSP; document with notes.
