Job summary

Care Lync seeks a Supports Coordinator (Early Intervention Services) to provide ongoing service coordination for children from birth to age 6 and their families. You will connect families with needed services, explain rights, and ensure coordinated, high-quality early intervention supports that promote child development and family capacity.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Early Childhood Education or related field.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

• Serve as primary point of contact for families and coordinate service delivery.
• Coordinate evaluations and assessments and facilitate IFSP development and reviews.
• Educate families on rights and safeguards, and promote caregiver empowerment.
• Monitor services for alignment with IFSP and coordinate across service systems.
• Maintain accurate records and complete service notes; ensure Medicaid/IDEA compliance.
• Collaborate with families, providers, and agencies and participate in team planning.
