Job summary

Care Lync is seeking a Supports Coordinator for Early Intervention Services in the United States. You will coordinate services for children birth to age 6 with developmental delays or disabilities and assist families in obtaining needed supports.

Responsibilities include intake, IFSP/FSP development, assessments, family education, and collaboration with a range of providers to ensure compliant service delivery in natural environments.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Early Childhood Education, Special Education, Child Development, Family Studies, Health-related field.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities; alternate education/experience combinations may apply.

Responsibilities

• Serve as primary point of contact for families and coordinate service delivery.
• Coordinate evaluations and planning, including IFSP/FSP development and reviews.
• Educate families on rights and safeguards; promote caregiver competence and empowerment.
• Monitor services for alignment with IFSP/FSP and coordinate across service systems.
• Maintain accurate records and complete service notes; ensure Medicaid and IDEA Part C compliance.
• Collaborate with families, providers, and agencies; participate in team planning.
