Job summary

Care Lync seeks a Supports Coordinator (Service Coordinator/Family Trainer) to provide ongoing service coordination for children birth to age 6 with developmental delays or disabilities and their families.

You will help families access services, understand their rights, and receive coordinated, high-quality early intervention supports. You will plan IFSP/FSP, monitor delivery, and document care while ensuring compliance with Medicaid and IDEA Part C.

Qualifications

• Bachelor's degree in a related field (Social Work, Psychology, Sociology, Early Childhood Education or related) or equivalent.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

• Serve as primary contact for families and coordinate access to services.
• Coordinate IFSP/FSP development and reviews with multidisciplinary teams.
• Educate families on rights and safeguards and promote empowerment.
• Monitor service delivery and ensure alignment with plans across systems.
• Maintain accurate service notes and documentation per regulatory requirements.
• Collaborate with families, providers, and agencies for coordinated care.
