Job summary

Care Lync is seeking a Supports Coordinator for Early Intervention Services in a community-based setting. You will coordinate services for children birth to 6 with developmental delays or disabilities and guide families to access necessary supports, understand their rights, and receive high-quality, coordinated services that promote child development and family capacity.

As the primary contact for families, you will coordinate evaluations, IFSP/FSP planning, and referrals, while collaborating

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Early Childhood Education, or related field.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities; alternative combinations of education and experience may apply.
• Experience with Early Intervention, IDEA Part C, or pediatric services preferred.

Responsibilities

• Serve as primary point of contact for families and assist in accessing needed services.
• Coordinate evaluations and assessments; support IFSP/FSP development and reviews.
• Educate families on rights, safeguards, and available supports.
