Job summary

Care Lync seeks a Supports Coordinator (Service Coordinator/Family Trainer) to provide ongoing service coordination for children birth to age 6 and their families, ensuring access to needed services and rights education within a coordinated, high-quality early intervention framework.

Under limited supervision, you will coordinate evaluations, develop IFSP/FSP, participate in multidisciplinary meetings, and educate families on rights. Travel and community-based work may be required.

Qualifications

• Bachelor's degree in a related field as listed.
• Minimum 1 year of experience with infants, toddlers, or children with disabilities.

Responsibilities

• Serve as primary point of contact for families and coordinate care across service delivery.
• Coordinate evaluations and assessments, and facilitate IFSP/FSP development and reviews.
• Educate families on their rights and options, promoting empowerment.
• Monitor services to ensure alignment with IFSP/FSP and coordinate across systems.
• Complete service notes and documentation to ensure Medicaid and IDEA Part C compliance.
• Collaborate with families, providers, and agencies to plan and communicate.
