Job summary

Care Lync seeks a Supports Coordinator for Early Intervention Services to provide ongoing service coordination for children birth to age 6 and their families. You will be the primary contact, help families access services, and ensure coordinated, high-quality supports that promote development and capacity.

You will coordinate evaluations, IFSP/FSP planning, and team meetings, educate families on rights, and monitor service delivery. Strong communication and collaboration with teams are essential.

Qualifications

• Bachelor's degree in one of the listed fields and related area.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.
• Experience with Early Intervention, IDEA Part C, or pediatric services is preferred.

Responsibilities

• Coordinate and monitor service delivery for families and children.
• Assist families in accessing needed services and understanding rights.
• Coordinate evaluations and IFSP/FSP development and reviews.
• Educate families, promote caregiver competence, and advocate for services.
• Maintain documentation and ensure compliance with Medicaid and IDEA Part C.
• Collaborate with families, providers, and agencies to plan and communicate.
