Job summary

Care Lync is seeking a Supports Coordinator for Early Intervention Services to provide ongoing service coordination for children birth to age 6 with developmental delays or disabilities and their families. You will help families access services, understand rights, and receive coordinated supports to promote child development.

The role covers coordinating evaluations, IFSP/FSR planning, multidisciplinary participation, and ensuring documentation and compliance across Medicaid and IDEA Part C,

Qualifications

  • Bachelor's degree in one of the listed fields or related area.
  • Minimum 1 year of experience working with infants, toddlers, or children with disabilities.
  • Current CPR/First Aid certification is preferred; candidates without certification will be provided support to obtain it upon hire.

Responsibilities

  • Serve as primary point of contact for families and coordinate service delivery.
  • Coordinate evaluations and assessments and facilitate IFSP/FSR development and reviews.
  • Educate families on their rights, advocate for family empowerment, and support services in natural environments.
  • Monitor services to ensure alignment with IFSP/FSR and coordinate across service systems.
  • Maintain accurate records and complete service notes in compliance with Medicaid and IDEA Part C.
  • Collaborate with families, providers, and agencies and participate in team planning.

Education

Bachelor's degree in Social Work, Psychology, Sociology, Early Childhood Education or related field
