Job summary

Care Lync is hiring a Supports Coordinator to provide ongoing service coordination for children from birth to age 6 and their families. You will help families access services, understand rights, and receive high-quality supports that promote development.

The role involves coordinating evaluations, IFSP/FSP planning, and multidisciplinary team participation while maintaining accurate documentation and ensuring compliance with Medicaid and IDEA Part C.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Early Childhood Education, or related field.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.
• Knowledge of IFSP/FSP development and child developmental milestones is preferred.
• Experience collaborating with families and community-based services.

Responsibilities

• Serve as primary contact for families and assist in accessing needed services.
• Coordinate evaluations, assessments, IFSP/FSP development and reviews.
• Monitor service delivery and ensure alignment with plans.
• Document service notes and maintain records.
