Job summary

Care Lync is seeking a Supports Coordinator to provide ongoing service coordination for children birth to age 6 with developmental delays or disabilities and their families. Under supervision, you will help families access services, understand rights, and coordinate, monitor, and document supports.

This role emphasizes collaboration with families, providers, and agencies, evaluating needs, developing IFSP/FSP plans, and ensuring compliance with Medicaid and IDEA Part C.

Qualifications

Responsibilities

• Serve as primary point of contact for families.
• Assist families in accessing needed services.
• Coordinate and monitor service delivery.
• Make referrals and schedule appointments.
• Conduct follow-up to ensure services are provided.
• Coordinate evaluations and assessments.
• Facilitate IFSP/FSP development and reviews.
• Participate in multidisciplinary team meetings.
