Job summary

Care Lync is seeking a Supports Coordinator (Early Intervention Services) to provide ongoing service coordination for children birth to age 6 and their families. You will ensure access to services, help families understand rights, and coordinate high-quality supports across service systems.

Responsibilities include serving as the primary contact for families, coordinating evaluations, and assisting with IFSP/FSP development, planning, and reviews.

Qualifications

• Bachelor's degree in one of the listed fields or related area.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

• Serve as primary point of contact for families and coordinate service delivery.
• Coordinate evaluations and IFSP/FSP development and reviews.
• Educate families on rights, safeguards, and advocacy.
• Monitor services for alignment with IFSP/FSP and coordinate across service systems.
• Maintain accurate records and complete service notes for compliance.
• Collaborate with families, providers, and agencies and participate in team planning.
