Job summary

Elk Valley, a part of LHC Group and member of the Optum family, is hiring a Home and Community Based Services Aide to support individuals to remain in their own homes with personal care and household tasks.

You will assist with daily living activities, monitor client well-being, document changes, and help with appointments. Requirements include a valid driver's license, reliable transportation, and TN-specific training; CPR and 6+ months home care experience preferred.

Qualifications

• Required: Current driver's license, vehicle insurance, and reliable transportation or access to public transit.
• TN: 75 hours of training including 16 clinical hours within first 3 months.
• CPR certification or ability to complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
