Job summary

Elite Hospice, part of the LHC Group and within the Optum family, seeks a Home and Community Based Services Aide to support clients at home. You will assist with daily living activities, monitor health, and help with transportation and errands, while maintaining confidentiality and safety.

The role emphasizes compassionate care, independent work, and flexible scheduling, with a focus on enabling clients to remain in their own homes and receive high-quality support.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation required.
• CPR certification preferred or ability to obtain within 90 days of hire.
• 6+ months of home care experience preferred.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
