Job summary

Elk Valley, part of the LHC Group and aligned with Optum, is hiring a Home and Community Based Services Aide in Tennessee. You will assist with personal care and daily living activities to help clients stay in their homes, with duties spanning hygiene, meal prep, errands, and transportation support.

Ideal candidates have a driver’s license, reliability, and some home care experience; CPR is a plus and flexible hours may be required.

Qualifications

Current driver's license, vehicle insurance, and reliable transportation or access to public transit.
TN: 75 hours of training including 16 clinical hours within first 3 months; qualified for basic services.
Preferred CPR certification or ability to complete within 90 days of hire.
6+ months of home care experience.
Ability to work flexible hours.

Responsibilities

Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
