Job summary

Elk Valley, a part of LHC Group within Optum, seeks a Home and Community Based Services Aide to support clients in their own homes across Tennessee. You will help with personal care and daily living tasks, enabling individuals to remain at home with dignity.

Responsibilities include documenting client condition, assisting with meals, transportation to appointments, and ensuring safety. The role offers a comprehensive benefits package with growth opportunities.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation.
• TN: 75 hours of training including 16 clinical hours within first 3 months.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Provide personal care and assist with bathing, grooming, dressing and other daily living activities.
• Support household tasks, meal preparation, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, incidents; maintain confidentiality and use EVV.
• Ensure a safe environment, operate medical equipment and respond promptly to client needs.
