Job summary

Elk Valley, a part of LHC Group within the UnitedHealth Group family, is looking for a Home and Community Based Services Aide to provide at-home personal care and support for individuals to live independently. You will assist with daily living activities such as bathing, grooming, and meal preparation, and accompany clients to appointments as needed.

State-specific training is required (TN: 75 hours with 16 clinical hours).

Qualifications

• Current driver’s license required with reliable transportation or access to public transit.
• CPR certification preferred or obtainable within 90 days of hire.
• 6+ months of home care experience preferred.
• TN-specific requirement: 75 hours of training with 16 clinical hours within first 3 months.

Responsibilities

• Provide personal care and assist with activities of daily living (bathing, grooming, dressing, ambulation, medication reminders).
• Assist with household tasks, meal preparation, and accompany clients to appointments or errands.
• Monitor and document client condition and vital signs; maintain confidentiality and use EVV system.
• Ensure a safe environment and respond promptly to client needs.
