Job summary

Elite Hospice, part of LHC Group within the Optum family, seeks a Home and Community Based Services Aide to help clients stay in their own homes. You will provide personal care, assist with daily living activities, and support household tasks with compassion and professionalism.

Responsibilities include monitoring client condition, documenting vital signs using EVV, ensuring safety, and respecting client preferences. CPR certification and reliable transportation are preferred.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
