Job summary

Elite Hospice, part of LHC Group and a member of the Optum family, is hiring a Hospice Home and Community Based Services Aide. You will support clients in their homes with personal care, daily living activities, and household tasks to help them remain in their own homes.

This role emphasizes safety, dignity, and compassionate care for diverse clients. Requirements include a valid driver’s license, transportation, and preferably 6+ months of home care experience with CPR or ability to obtain

Qualifications

• Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
• 6+ months of home care experience.
• CPR certification or ability to obtain within 90 days.
• TX: Home Health Aide experience or nursing student with competency documented within last 24 months.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal prep, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
