Job summary

Kona Medical Consulting is seeking a Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care in patients' homes within the Dallas–Fort Worth area. The ideal candidate is reliable, organized, and confident in clinical decision-making.

You will perform home visits, update care plans, manage wounds and medications, educate patients, and coordinate with physicians and care teams. Flexible scheduling and supportive leadership await.

Qualifications

• Current Texas RN license
• 1+ year nursing experience (Home Health preferred)
• Strong clinical assessment and critical-thinking skills
• Reliable transportation, valid driver’s license, and proof of auto insurance
• Comfortable using EMR/mobile charting
• Compassionate, professional, and patient-focused

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families
