Job summary

Kona Medical Consulting is seeking a Home Health Field Registered Nurse (RN) to deliver skilled nursing care in patients’ homes within the Dallas–Fort Worth Metroplex. The role emphasizes assessments, care planning, wound care, and patient education, with a focus on compassionate, patient-centered service.

The ideal candidate will be Texas-licensed with strong clinical skills, comfortable using EMR/mobile charting, and able to work independently.

Qualifications

• Current Texas RN license required.
• 1+ year nursing experience; Home Health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Provide wound care, disease management, meds administration, and patient education.
• Communicate with physicians, case managers, and teams.
• Complete timely documentation per regulatory requirements.
• Participate in on-call rotation as needed.
• Build trust with patients and families.

Position Overview
