The ideal candidate has a Texas RN license, 1+ year home health experience, strong assessment skills, and reliable transportation. You will develop care plans, perform wound care, educate patients, and collaborate with physicians and case managers.

Qualifications

• Current Texas RN license.
• 1+ year of nursing experience; Home Health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• In-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Wound care, disease management, medication administration, and patient education.
• Communicate with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation per regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
