Kona Medical Consulting, in partnership with The Home Health Team, seeks a Home Health Field Registered Nurse (RN) to deliver skilled nursing care in patients’ homes across the Dallas–Fort Worth Metroplex. You will assess, plan, and implement care with a focus on outcomes and safety.

The ideal candidate has 1+ year of nursing experience (Home Health preferred), strong clinical skills, reliable transportation, and comfort with EMR/mobile charting.

Qualifications

• Current RN license in Texas.
• 1+ year of nursing experience (Home Health preferred).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
