Kona Medical Consulting is actively seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care to patients in their homes within the Dallas–Fort Worth Metroplex. The role demands reliable, organized, and clinically confident professionals who are passionate about helping patients reach their health goals.

You will perform in-home assessments, develop care plans, provide wound care and education, and collaborate with physicians and care teams.

Qualifications

• Current RN license in Texas.
• 1+ year of nursing experience (Home Health preferred).
• Home Health experience preferred but not required.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
