Job summary

OneWell Health Care is seeking a compassionate Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will assist with daily living activities, health and safety monitoring, and skill development under ISP/IPP plans.

This field-based role requires reliable transportation and strong documentation, with HIPAA compliance and teamwork.

Qualifications

High school diploma or equivalent required.
Ability to use electronic documentation and EVV systems.
English proficiency in spoken and written form.

Responsibilities

Provide consistent, respectful, high-quality support in accordance with each individual’s care plan.
Assist with personal care and daily living activities; support meal preparation, nutrition, mobility, and medication assistance as authorized.
Implement behavior support strategies; monitor health, safety, and emotional well-being.
Promote independence, stability, and community participation; maintain routines; assist with transportation to appointments or community outings.
Participate in trainings and team meetings; maintain documentation; provide flexible support during staffing shortages per program needs.
