Job summary

OneWell Health Care is seeking a dedicated Caregiver / Direct Support Professional (DSP) to support individuals with IDD in home and community settings across Pennsylvania. This field-based role offers flexible scheduling and requires adherence to HIPAA and organizational policies.

As a DSP, you will assist with daily living activities, monitor health and safety, and provide transportation and community inclusion support.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support for individuals with IDD preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide consistent, respectful, and high-quality support per care plan.
• Assist with personal care and daily living activities; support meal prep, mobility, and medication assistance.
• Document accurately with daily service notes and participate in trainings and team meetings.
