Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide person-centered care to individuals with IDD in home and community settings. You will support daily living activities, skill development, and community participation while following ISP goals and Virginia HCBS standards.

The role emphasizes safety, confidentiality, documentation, and collaboration with the IDD Team Lead to ensure quality of life and independence for clients.

Qualifications

• High school diploma or equivalent required.
• English proficiency for spoken and written communication.
• Ability to use EVV systems and basic computer apps.

Responsibilities

• Provide direct, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, mobility, safety, and ISP goals.
• Document observations, incidents, and care activities accurately and promptly.
• Follow safety procedures, HIPAA rules, and program policies.
• Assist with trainings, teamwork, and transportation when needed.
