Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) to provide person-centered, community-based support for individuals with IDD and Autism in Connecticut. The DSP will assist with daily living activities, skill development, health monitoring, and community participation while upholding dignity and independence in alignment with regulatory standards.

The role reports to program leadership and involves working in residential homes, community settings, and day programs

Qualifications

• High School Diploma or GED required.
• English proficiency (spoken and written) required.
• Valid Driver's License and reliable transportation preferred.

Responsibilities

• Provide person-centered support to individuals with IDD and Autism.
• Assist with activities of daily living and skill development.
• Implement ISPs and BSPs and monitor health and safety.
• Maintain accurate and timely documentation (daily notes, MARs, incident reports).
• Collaborate with interdisciplinary teams and participate in trainings.
