Job summary

OneWell Health Care in Connecticut seeks a dedicated Direct Support Professional (DSP) to provide person-centered, community-based support to individuals with IDD and Autism in residential homes, community settings, and day programs.

You will assist with daily living activities, support ISP and BSP plans, monitor health and safety, maintain documentation, and collaborate with interdisciplinary teams to promote independence and inclusion while adhering to state and agency standards.

Qualifications

• High school diploma or GED is required.
• Minimum age 18 years.
• Experience working with IDD and behavioral support preferred.
• Experience in community-based services preferred.

Responsibilities

• Provide person-centered support to individuals with IDD and Autism.
• Assist with activities of daily living and health monitoring.
• Maintain accurate documentation including MARs and progress notes.
• Collaborate with interdisciplinary teams and participate in trainings and meetings.
