Job summary

CVS Health is seeking a Registered Nurse Case Manager in New Jersey to coordinate care for members with complex needs, helping them live safely at home. This field-based role combines independence with meaningful community impact, with travel within the county and surrounding area reimbursed.

You’ll perform in-home assessments, collaborate with providers, develop care plans, and support transitions of care.

Qualifications

• Active, unrestricted RN license in New Jersey.
• At least 3 years of clinical experience.
• Comfort with frequent local travel and in-home visits.
• Minimum NJ Choice Certification requirements met.
• Ability to work independently and manage a flexible daily schedule.

Responsibilities

• Conduct in-home assessments for members enrolled in MLTS programs.
• Evaluate medical, behavioral, and social needs to support appropriate care.
• Collaborate with providers and interdisciplinary teams for coordinated care.
• Develop and manage individualized care plans promoting safety and independence.
• Document assessments and care activities in the EHR.
• Support care transitions and connect members with community resources.
• Mentor new team members as you grow in the role.
