Job summary

Chapters Health System is seeking an RN Case Manager for nights and weekends to perform after-hours field visits across assigned territories. You will build strong relationships with patients, families, and referral sources while delivering high-quality hospice care.

Requirements include an active RN license in Florida, at least one year of nursing experience, and proficiency with EMR/EHR systems. Bilingual in Spanish is a plus; valid driver’s license and reliable transportation are required.

Qualifications

• Current license as RN in the state of employment.
• Minimum of one year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Ability to communicate effectively in English; bilingual in Spanish preferred.

Responsibilities

• Makes after-hours field visits to patients/families and clinical teams.
• Provides direct patient care and coordinates with the IDG and referral resources.
• Documents accurately per policy and procedures; communicates changes.
• Shows compassion, clinical judgment and teamwork in a fast-paced home/hospice setting.
