Job summary

Chapters Health System is seeking an RN for Night and Weekend coverage to perform after-hours field visits across patient homes and facilities in Florida. You will assess needs, coordinate care with the interdisciplinary team, and ensure quality outcomes for patients and families.

The role requires strong clinical skills, compassion, and the ability to work independently in a fast-paced environment while maintaining patient safety and satisfaction.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience using an EMR/EHR system.
• Ability to communicate in English (verbal and written); bilingual in Spanish preferred.
• Mobile driver with valid license and automobile insurance per company policy.
• Reliable transportation to meet patients’ needs.

Responsibilities

• Makes after-hours home visits to patients within assigned territory; provides compassionate customer service.
• Assesses patient/family needs and initiates appropriate nursing interventions by phone or in person.
• Delivers direct care as part of the IDG, addressing psychosocial, spiritual, cultural, and physical factors.
• Demonstrates clinical skills in case management, physical assessment, IV therapies, pain titration, and safety.
