Qualifications

• RN license in the state of employment.
• At least 1 year nursing experience.
• Hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Bi-lingual in Spanish preferred.
• Valid driver’s license and transportation.

Responsibilities

• Make after-hours home visits to patients and families.
• Establish and maintain relationships with care teams and referral resources.
• Provide direct nursing care and case management as part of IDG.
• Document care per policy and procedure.
