Chapters Health System, Inc. is seeking an RN for Nights and Weekends to perform after-hours visits to patients and families, coordinating care and ensuring high-quality patient service in Florida.

The role requires active RN licensure in Florida, 1+ year nursing experience (hospice/hospital preferred), and proficiency with EMR/EHR systems; bilingual Spanish is a plus. A valid driver’s license and reliable transportation are required.

Qualifikationen

• Active RN license in the state of employment (Florida).
• Minimum of 1 year nursing experience; hospice or hospital setting preferred.
• Experience with EMR/EHR systems for patient records and documentation.
• Fluent English communication; Spanish bilingualism preferred.
• Valid driver’s license with reliable transportation for home visits.

Aufgaben

• Make after-hours visits to patients in homes, ALFs, nursing homes, or hospitals within assigned territory.
• Assess patients/families and determine immediate nursing needs; provide appropriate interventions.
• Deliver direct care as part of the IDG, addressing physical, psychosocial, and spiritual needs.
• Document thoroughly; communicate with physicians and care teams; initiate plans of care.
• Prioritize tasks, maintain professionalism, and ensure timely follow-up and safety during emergencies.
