Job summary

Chapters Health System is seeking an experienced RN for night and weekend shifts to provide after-hours visits to patients and families in the Lakeland area. You will work with an interdisciplinary team, document in the EMR, and coordinate care with physicians and community partners.

Ideal candidates hold an active Florida RN license, with hospice or hospital experience preferred, and strong communication and urgent decision-making skills.

Qualifications

• Active RN license in Florida.
• Minimum of 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong communication skills in English; Spanish bilingual a plus.

Responsibilities

• Make after-hours visits to patients and families in homes, facilities, or hospitals.
• Assess patient needs and provide direct nursing interventions as part of IDG.
• Document care in the EMR per policy and ensure timely follow-up with IDG.
• Coordinate with physicians, facilities, and community partners to ensure continuity of care.
