Job summary

Chapters Health System is seeking a Registered Nurse for night and weekend coverage to perform after-hours field visits to patients, families, and community partners across the assigned territory in Florida.

You will assess needs, provide direct patient care, and collaborate with the IDG to ensure compassionate, high-quality care while maintaining documentation and regulatory compliance. Strong communication, critical thinking, and a calm approach to stressful situations are essential.

Qualifications

Current RN license in the state of employment.
Minimum of one year nursing experience; hospice or hospital preferred.
Experience with EMR/EHR systems.
Ability to communicate in English (verbal and written); bilingual in Spanish preferred.
Valid driver’s license and vehicle insurance per company policy.

Responsibilities

Makes after-hours visits to patients in home, facility, or hospital within territory.
Assesses patient/family and identifies immediate needs and nursing interventions.
Provides direct care as part of IDG, including psychosocial and physical care.
Demonstrates clinical skills in case management, assessment, IV therapies, and symptom management.
Prioritizes, refers, and collaborates with other professionals to ensure quality care.
Educates patient and family about disease process, care options, and safety practices.
