Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician to manage complex member cases across the care continuum. This field-based role travels to client sites and designated locations, with periodic office attendance as needed.

You will assess, plan, implement, and monitor individualized care plans for members with chronic conditions, collaborating with the health team to optimize health outcomes. A valid Ohio RN license and relevant experience are required.

Qualifications

• Requires a high school diploma or GED and a minimum of 3 years' experience with chronic illnesses or disabilities in a case management-like role.
• Current, active RN license in Ohio state required.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify and coordinate member needs.
• Manage care for members with chronic illnesses to ensure cost-effective benefit use.
• Obtain thorough member histories to develop individualized care plans.
• Set short and long-term goals with members, caregivers and physicians.
• Develop care plans and ensure access to services.
• Assist with authorizations/referrals within benefits or waivers as allowed.
• Coordinate with Medical Directors and interdisciplinary teams on care plans.
