Job summary

Elevance Health seeks a field-based LTSS Service Coordinator – RN Clinician to manage members' care across physical health, behavioral health, and long-term supports. Travel to client sites will be required, with occasional office attendance as needed.

You will develop and monitor personalized care plans, coordinate with the health team, and optimize access to services while prioritizing person-centered care. A valid Ohio RN license and prior case management experience are essential.

Qualifications

• Requires a high school diploma or GED and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role; or any combination of education and experience, which would provide an equivalent background.
• Current, active valid and unrestricted RN license in Ohio state(s) required.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify and manage members' needs across physical and behavioral health, social services, and long-term supports.
• Coordinate care for members with chronic illnesses to ensure cost-effective and efficient utilization of health benefits.
• Develop individual care plans with short and long-term goals in collaboration with members and care teams.
• Facilitate authorizations/referrals and assist in implementing member care plans within benefits structure.
• Interface with Medical Directors and interdisciplinary teams on care management and problem-solving with providers.
