Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician to manage member cases within licensure scope, develop and monitor care plans, and coordinate services across physical health, behavioral health, long-term supports, and psychosocial needs. This field-based role travels to client sites throughout Ohio.

You will perform functional assessments, collaborate with care teams, and ensure access to services, with preference for candidates already residing in the service areas.

Qualifications

• Minimum requirement: high school diploma or GED and 3 years’ experience with chronic illnesses or disabilities in a case management or similar role.
• Active, unrestricted RN license in Ohio required.

Responsibilities

• Perform telephonic and in-person assessments to identify member needs.
• Develop, monitor and revise individualized care plans.
• Coordinate services with the health care team and providers.
• Identify high-risk members and optimize care across the continuum.
• Facilitate referrals and authorizations within benefits.
• Engage with members, families and physicians to meet goals.
• Ensure care plans comply with licensure and policies.
