Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician (Case Manager) to travel to client sites and coordinate comprehensive care. You will assess, plan, and monitor care across physical health, behavioral health, and long-term services, collaborating with physicians and care teams.

This field‑based role requires an Ohio RN license and experience with chronic illnesses, co-morbidities, and disabilities. Hybrid work options may apply.

Qualifications

• Requires a high school diploma or GED and at least 3 years in case management or similar with chronic illnesses.
• Current, active valid and unrestricted RN license in Ohio.
• BA/BS in Health/Nursing preferred.
• Experience with older adults or disabilities preferred.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify and manage member needs.
• Coordinate care for physical health, behavioral health, and long-term services.
• Develop and monitor individualized care plans with the member and care team.
• Collaborate with Medical Directors and interdisciplinary teams for care management.
• Assist with authorizations/referrals within benefits where appropriate.
