Job summary

Elevance Health seeks an LTSS Service Coordinator – RN Clinician (Case Manager) to manage member cases in the field across multiple counties in Ohio. This role requires developing, monitoring, and revising care plans to meet individual needs, with a focus on person-centered care.

The position emphasizes telephonic and in-person assessments, collaboration with care teams, and coordination of services and waivers when appropriate. Travel is involved; hybrid work options may apply.

Qualifications

• Requires a high school diploma or GED and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in Ohio state(s) required.
• Preferred: BA/BS in Health/Nursing; case management experience with older adults or individuals with disabilities.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify, evaluate, coordinate and manage member needs (physical health, behavioral health, social services, LTSS).
• Manage members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of health benefits.
• Obtain thorough member history to develop an individualized care plan.
• Establish short and long-term goals with the member and supporters; identify candidates for alternative levels of care or waiver programs.
• RN has responsibility to develop care plans and ensure access to services.
• Assist with implementation of care plans by facilitating authorizations/referrals as appropriate.
• Interface with Medical Directors and care teams on development of person-centered care plans.
