Job summary

Elevance Health seeks an LTSS Service Coordinator – RN Clinician (Case Manager) to travel to client sites and coordinate long-term services and supports. The role emphasizes trauma-informed, person-centered care and collaboration with healthcare teams to address physical, behavioral health, and social needs.

The position requires an active Ohio RN license and a minimum of 3 years in case management or similar roles.

Qualifications

• Requires RN licensure in Ohio and experience with individuals with chronic illnesses or disabilities.
• Field-based case management and care coordination experience.
• Strong assessment, collaboration with clinicians, and care planning skills.

Responsibilities

• Perform telephonic and in-person functional assessments to identify and manage member needs.
• Develop and monitor member care plans with goals in collaboration with the member and care team.
• Coordinate authorizations/referrals and interface with medical directors and teams.
