Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum of 3 years of clinical experience in hospitals, home health, or ambulatory care.
• Residency in Middlesex County, NJ.
• Willingness to travel up to 75% of the time; mileage reimbursed per policy.
• Successful completion of NJ Choice Certification (80%+ on exam; state training modules; field mentoring).

Responsibilities

• Conduct face‑to‑face home visits using assessment tools for MLTSS and D‑SNP/FIDE.
• Perform in‑person assessments for non‑MLTSS members to determine medical needs and referrals.
• Coordinate with providers, authorized representatives, and care teams.
• Develop individualized care plans and authorize services within MLTSS/FIDE benefits.
• Document assessments and care planning in the electronic health record.
