Qualifications

Responsibilities

• Conduct face-to-face home visits for ML TSS and D-SNP/FIDE members.
• Perform in-person assessments for non-MLTSS members to determine needs.
• Coordinate with providers and interdisciplinary care teams.
• Document assessments and care planning in EHR.
